Pensando la determinación social del proceso salud-enfermedad / Thinking about the social determination of the health-disease process

RESUMEN El artículo presenta la discusión epistemológica que subyace a la propuesta de la determinación social del proceso salud-enfermedad, enmarcando dicha discusión en la historia de la corriente de pensamiento llamada Medicina Social Latinoamericana y analizando la manera como dicha corriente plantea el estudio de los procesos de salud-enfermedad en las sociedades contemporáneas. Para tal fin, se elabora inicialmente un breve marco histórico luego se analizan las implicaciones epistemológicas del concepto de determinación social del proceso salud-enfermedad y, por último, se ilustra con tres ejemplos la manera como se aplican las ideas acerca de la determinación en el estudio concreto de la realidad.(AU)

ABSTRACT This article presents an epistemological discussion that underlies the proposal of social determination of the health-disease process, framing this discussion in the history of the current of thought known as Latin American Social Medicine, and analyzing the way in which this current approaches the study of health-disease processes in contemporary societies. To this end, a brief historical framework is elaborated, and the epistemological implications of the concept of social determination of the health-disease process are analyzed. Finally, three examples illustrate how the ideas about determination in the concrete study of reality are applied.(AU)

[Thinking about the social determination of the health-disease process]. / Pensando la determinación social del proceso salud-enfermedad.

This article presents an epistemological discussion that underlies the proposal of social determination of the health-disease process, framing this discussion in the history of the current of thought known as Latin American Social Medicine, and analyzing the way in which this current approaches the study of health-disease processes in contemporary societies. To this end, a brief historical framework is elaborated, and the epistemological implications of the concept of social determination of the health-disease process are analyzed. Finally, three examples illustrate how the ideas about determination in the concrete study of reality are applied.

El artículo presenta la discusión epistemológica que subyace a la propuesta de la determinación social del proceso salud-enfermedad, enmarcando dicha discusión en la historia de la corriente de pensamiento llamada Medicina Social Latinoamericana y analizando la manera como dicha corriente plantea el estudio de los procesos de salud-enfermedad en las sociedades contemporáneas. ¿Determinación social o determinantes sociales?: Diferencias conceptuales e implicaciones praxiológicas / Conceptual differences and praxiological implications concerning social determination or social determinants

Las diferencias entre la determinación social y los determinantes sociales de la salud no son sólo conceptuales, son también ético-políticas. Conllevan rutas y apuestas divergentes, explicaciones distintas sobre la causalidad y el riesgo en salud con implicaciones esenciales en las relaciones de poder, en la concepción de la ética y en la comprensión del proceso salud enfermedad. El propósito de este texto es advertir las implicaciones praxiológicas de los enfoques de la epidemiología social latinoamericana y la anglosajona y, nutrir el debate entre la determinación social y los determinantes sociales de la salud. Para ello primero sitúa la construcción socio-histórico de estos enfoques, luego analiza sus diferencias con base en criterios de valoración epistemológica, metodológica y ontológica y finaliza con una breve consideración acerca de las implicaciones prácticas que conllevan estas diferencias.

The differences between the social determination of health approach adopted by the Latin-American Social Medicine and Collective Health movement and the WHO's social determinants of health approach are not merely conceptual but involve ethical and political considerations. Different notions of causality and risk are implied in the aforementioned approaches and shape how concepts regarding health-illness and health inequity are understood and how they may be confronted. This article attempts to clarify the praxiological implications of such approaches and contextualise the approaches' socio-historical construction, address epistemological, methodological and ontological differences and propose some considerations regarding the praxiological implications.

The differences between the social determination of health approach adopted by the Latin-American Social Medicine and Collective Health movement and the WHO's social determinants of health approach are not merely conceptual but involve ethical and political considerations. Different notions of causality and risk are implied in the aforementioned approaches and shape how concepts regarding health-illness and health inequity are understood and how they may be confronted. This article attempts to clarify the praxiological implications of such approaches and contextualise the approaches' socio-historical construction, address epistemological, methodological and ontological differences and propose some considerations regarding the praxiological implications.

Modalidades de evaluación de las iniciativas latinoamericanas promotoras de la práctica de la lactancia materna / Ways of evaluating Latin-American initiatives for promoting breastfeeding

En este trabajo se analizan las diferentes modalidades utilizadas para evaluar las iniciativas que promueven la práctica de la lactancia materna, a partir del año 1990 en América Latina. Para cumplir con este propósito, se llevó a cabo una revisión de la literatura basada en un protocolo de búsqueda, selección y análisis de las diferentes experiencias de evaluación. Se seleccionaron 58 evaluaciones para su estudio y comparación. Entre los hallazgos, cabe señalar que los tipos de evaluación más usados fueron los de efectividad y de impacto y la evaluación se hizo, en especial, a programas educativos o de consejería en lactancia materna. Igualmente, las evaluaciones se realizaron a iniciativas que contemplaban dos o tres estrategias de la Promoción de la Salud, relacionadas con el desarrollo de habilidades personales y la reorientación de servicios de salud. Los resultados obtenidos ofrecen una visión general de la manera en que son evaluadas las iniciativas que promueven la práctica de la lactancia materna, a la luz de la literatura existente acerca de la evaluación en Promoción de la Salud.

This work has analyzed the different methods used since the 1990s for evaluating initiatives aimed at promoting breastfeeding in Latin-America. A review was made of the pertinent literature using a protocol based on researching, selecting and analyzing different ways of making such evaluation. Fifty-eight evaluations were selected for study and comparison. It is worth noting that the most commonly used types of evaluation concerned initiative effectiveness and impact; assessment was particularly aimed at educational or breastfeeding counseling programs. Evaluation also concerned initiatives encompassing two or three healthcare promotion strategies related to developing personal skills and the reorientation of healthcare services. The results provided an overview of the way initiatives promoting breastfeeding have been evaluated related to the pertinent literature regarding how healthcare promotion is evaluated.

Repensando la promoción de la salud en el Sistema General de Seguridad Social en Salud / Rethinking how health is promoted in the Colombian general health-related social security system (Sistema General de Seguridad Social en Salud)

El artículo presenta un balance acerca de la manera como se ha venido entendiendo la Promoción de la Salud (PS) en el ámbito internacional y señala que, cuando menos, existen tres distintas acepciones de la promoción: la primera como orientación general de política, la segunda como conjunto de acciones y la tercera como una dimensión especial del trabajo sanitario. En medio de las sutiles diferencias que se establecen desde estas diversas maneras de asumir la PS, surge el interés expreso por darle una fundamentación específica que la distinga de la prevención y que trascienda la labor educativa. Una vez reconocido el reto que plantea la PS en el campo sanitario, el texto describe de forma sucinta, cómo se introduce el discurso de la PS en Colombia y discute la forma como se ha intentado darle concreción dentro del Sistema General de Seguridad Social. Por último, se hace mención de los esfuerzos que actualmente se están haciendo en el país dirigidos a hacer de la Promoción de la Salud una realidad en el sistema sanitario colombiano.

This article takes stock of how promoting health (PH) (promoción de la salud - PS) has been understood within an international setting and gives three meanings for promotion: as general policy orientation, as a set of actions and as a special dimension of sanitary work. Interest expressed in giving it a specific basis distinguishing it from prevention, transcending educational work, has emerged from the subtle differences established from such different ways of coming to terms with PH. After recognising the challenge posed by PH in the field of health, the text succinctly describes how discourse regarding PH has been introduced in Colombia and discusses how an attempt has been made to be more precise regarding its place in the general health-related social security system. Efforts currently being made in Colombia regarding making PH become a reality within the Colombian health system are then mentioned.
